Please complete this form if you are going to miss a general assembly meeting, committee meeting, office hour or event.

Please complete this form by noon of the same day for general assembly meetings, committee meetings or events. Please complete this form by 10PM the night before for office hours. If you are going to be absent for a tabling hour or office hour where tickets are being sold for an event and there is no one else working that shift, please call an executive board member, in addition to filling out this form.

Please note that completing this form does not necessarily excuse your absence.
It will be decided by the Executive Vice President and recorded by the VP of IA.
